12/11/2023 EnchantaVaughn Approved Behavior: Client was observed during the session with an upset look on her face and choosing to distance herself away from her counselor when inquired about why she didn’t want to talk or share anything today. Client was given a few minutes to collect herself and get back on track with achieving her objectives. Client expressed she was upset at her mother who screamed at her and sent her to her therapy care. Client continues to display attention-seeking behaviors but was able to reduce negative tension and act calmer in the session. Behavior: Client was assisted with creating and making her ideas come to life so that she would feel less exhausted and more motivated to speak up about her personal challenges. Client stated that she hasn’t shown the best attitude in the program this week but would do better moving forward. Client was able to partially achieve her objectives due to being respectful to staff when receiving the help that she needed, controlling her emotions and remaining calm, despite her feeling mistreated earlier. Intervention: QMHP will collaborate with parent/close family member regarding behaviors and performance. QMHP intervened in an effort to encourage client to make the right decision. QMHP informed client that refusing to listen to her mom was not acceptable and will not be tolerated. QMHP redirected client immediately. QMHP encouraged client to make good choices and to utilize self-calming techniques (i.e. counting and deep breathing exercises) to teach client a safe way to respond to the conflicts at home. Intervention: QMHP discussed actions and consequences. QMHP will teach client necessary coping skills to decrease impulses significantly. QMHP encouraged client to take small breaks and then process forward. QMHP encouraged client to be mindful of others and utilize role reversal techniques to put herself in their shoes. QMHP monitored client until the end of session to make sure she was obedient and respectful to her family. QMHP applauded client for her efforts displayed in session today. Response: Client transitions well between one on one discussion and family discussions, utilizing the correct voice volume and displaying positive body language. Client was observed adhering to rules and being respectful to VACCD staff. Client displayed self-control by keeping her hands to herself and sitting calmly without impulsive or hyperactive behaviors. Client agrees to follow directions even if she sees someone else breaking them. Client demonstrates she can accept constructive criticism and act civilized. Response: Client completed her activity without any interruptions. Client was acceptive of prompts and encouragement given by QMHP. Client was praised for her cooperation and modeling appropriate behavior. Client nodded in agreement and promised to do what was asked by her mother and mental health professional. Client appeared to be learning from her mistakes and correcting them throughout the day. Client was happy to know she was improving and meeting her objectives as best as she could.
